I see what you are saying with the header missing from printing straight from the list.
For the time being, if you needed the headers to print out you can export the sheet to excel.

ParticipantI apologize for the frustration this is causing both schools.
Regarding the export “all” not working. Try this, go down to the number of records selector at the bottom of the list and expand it to all. Then click the select all and then group actions > export. I think this will work for you.
Regarding the extra information in the spreadsheet. I see exactly what you are talking about and I will bring the issue to the development team.
